Social Prescribing
Social prescribing is all about connecting people with non-medical support that can make a real difference in their everyday lives — things like meeting new people, learning new skills, or getting help with finances. We believe everyone deserves to feel heard and supported, not just in the consultation room but in the wider world too.
No matter your walk of life, social prescribing can offer a way forward. It’s an effective approach that puts people at the heart of health and well being.
Community Support
We work with community groups and organisations to connect you to support in the place you live. Accessing help locally is especially important for anyone feeling isolated, anxious or unsure where to turn.
Through Health Exchange, you can explore:
- Befriending and peer support groups
- Arts, gardening, and exercise sessions
- Help with money or housing issues
- Volunteering, employment and learning opportunities
- Local faith or cultural groups
We offer flexible appointments and work in friendly, easy-to-access community spaces. We’re here to make it easier to get support in the right place and at the right time.


Link Workers
Our social prescribing link workers are trained professionals who will listen, guide, and connect you with community-based support.
They work closely with GPs and healthcare teams as part of Primary Care Networks (PCNs). This means you can be put in touch with a social prescribing service through your GP practice.
Here’s what our link workers do:
- Build trusting, one-to-one relationships
- Look at the whole picture — not just medical issues
- Co-create simple, personalised wellbeing plans
- Offer encouragement and follow-up support
Children and Young People
Growing up isn’t always easy. Many children and young people face challenges — whether that’s anxiety, loneliness, stress at school or problems at home. Sometimes, these issues can feel too big to manage alone.
Our social prescribing services support young people to feel more connected, confident and cared for.
This might include introducing them to:
- Youth groups and mentoring
- Support for young carers
- Art and nature-based activities
- Emotional wellbeing programmes
- Support in combining healthy eating, physical activity, sleep, mental wellbeing, and digital safety


Secondary Care
The time after a hospital stay can often bring its own set of challenges. Our social prescribing team can connect you with local support to help get you back on your feet. We will work with you to find ways to recover and reduce your chance of readmission.
If you have complex needs or have been in and out of hospital, we’re here to offer you options.
Health and Wellbeing Coaching
Our health and wellbeing coaches can help you make meaningful lifestyle changes. This might include getting more active, managing an existing health condition, or building healthier habits.
Coaching isn’t just about giving advice — our supportive coaches are with you every step of the way, using proven techniques to build confidence and motivation.
With the help of a health and wellbeing coach, you can:
- Take steps toward the life you want
- Set realistic goals
- Build lasting habits
- Learn how to manage health challenge
