Embedded within primary care, our link workers offer structured non-clinical support fully aligned with PCN ARRS roles.
Hospital Wellbeing & Social Prescribing
We integrate social prescribing into urgent and secondary care pathways to support discharge, prevent readmission, and address wider social care and health determinants.
Community Wellbeing & Social Prescribing
Our community model provides person-centered, strength-based support embedded in local health and care systems.
